HHI offers a PDPM tool kit to assist our clients with the overall day-to-day management of patient care and reimbursement. Under the PDPM model, it is so important to maintain an interdisciplinary approach, recognize acuity levels and identify patient goals.

Patient assessments need to start with the initial referral and should remain ongoing throughout the skilled admission. At a minimum, all active diagnoses need to be reviewed, nursing documentation needs to reflect skilled levels of care, baseline care plans must be established, and patient function and interviews need to be completed. Together, in doing this, we are better able to manage the necessary care for our patients to optimize patient outcomes. 

Facilities need to assess and understand patient acuity for each patient to ensure proper reimbursement for patient care.
